Requisitions Scan Sheets

Stakeholders require us to recharge work that we produce, which means that we must ask you to complete a request form (requisition) whenever you order work from us.

The following requisitions must be saved on your PC prior to use.

To download a requisition to your PC, please:

• Click on the appropriate link below
• When the document has opened fill in your details in “Information about you”
• Save the requisition to your computer as a document template. The requisition is now ready for use.

Using the requisition

1. Open the file (If you are asked, enable macro’s)

2. Complete the requisition

3. Send the completed requisition to JRS@JRS.ac.uk as an attachment via your email System 

Your completed requisition is in the attachments part of the email and if you are submitting a job at the same time, please attach it now.

When we receive your requisition we will email you with a job reference number.
